Case management at Eden’s Farm offers holistic, survivor-centered support designed to help you navigate life’s challenges and take steps toward your personal goals. Through regular check-ins, referrals, and planning sessions, our case managers can assist with:
Resource Navigation
ID recovery, medical care, mental health services
SNAP/benefits applications, transportation assistance
Legal aid referrals and expungement clinics
Goal and Planning Support
Short- and long-term goal setting
Emotional and practical support along the way
Education and Employment
GED and continuing education referrals
Resume building, job readiness, and interview prep
Basic Needs Support
Hygiene kits, clothing, food access referrals
Safety planning and crisis support
Referrals to Trauma Informed Services
Therapy, support groups, substance use programs
